Worksheet 1

Official dates and notices

  • I have the most recent written hearing notice.
  • I recorded the date, time, format, and source of that information.
  • I listed any deadline or response instruction exactly as written.
  • I know where to verify the schedule with BPH.

Questions or missing information

Worksheet 2

Counsel and records

  • I know who counsel is and how permitted communication works.
  • I listed the records already reviewed.
  • I listed records or notices that appear to be missing.
  • I separated factual questions from legal questions for counsel.
  • I marked anything I do not understand instead of guessing.

Documents reviewed or still needed

Worksheet 3

Support and practical planning

  • Housing information is current and confirmed.
  • Transportation steps identify who, where, and when.
  • Health, identification, benefits, or program needs are listed.
  • Support contacts have agreed to the role described.
  • Any written support is truthful, specific, and based on personal knowledge.
Specific is more useful than vague. Instead of “transportation is handled,” record who has agreed to help, the first destination, and what still needs confirmation. This is an organization example, not a recommended answer.

Confirmed support and open questions

Worksheet 4

Hearing day and after

  • I confirmed the current hearing format and access instructions.
  • I wrote down accessibility or communication needs to raise through the proper process.
  • I know where allowed materials and personal notes will be kept.
  • I have a plan to keep the written decision and notices together.
  • I will review case-specific next steps with counsel.
